Paramedic Use

Thank you for your interest in using the Wong-Baker FACES® Pain Rating Scale for paramedic use.  Please read the following information and submit the form below.  We will review your information and send you digital images of our scale along with access to resources and our growing number of language translations.  This permission is not extended to the facilities you serve.

Guidelines for Paramedic Use

Help us maintain the integrity of the Wong-Baker FACES® Pain Rating Scale by following these simple guidelines:

  • Remember, our scale is used for pain assessment, only. We do not give permission for use in measuring emotions, moods, comfort, etc. This is essential for our scale’s validity. You will find the Instruction Guide for using the Wong-Baker FACES® Pain Rating Scale on the Downloads page that will follow after you submit the form below.
  • Ours is a self-assessment scale, so the patient needs to be able to understand the instructions and select a face that illustrates the pain they are experiencing. It should never be used with unresponsive patients or to compare the patient’s face to the scale to determine the level of pain.
  • No modifications should be made to this scale without our expressed permission. We are intentional about not adding color to our scale, because color is so subjective. If you see a scale with color added, that is a copyright infringement.

You should receive an automatic email response to your web form submission and an opportunity to confirm your email address.  If you do not receive communication, chances are it has been delivered to your junk file or trash or perhaps not delivered at all.  Please email us at info@WongBakerFACES.org, if that is the case and we will send your information another way.
